The usual techniques to repair the septum depend on the problem with the septum. Obstructing bone and cartilage is either removed or moved back to the midline with care to leave enough support so that the bridge does not collapse. Usually the nose is splinted with an internal silicone splint rather than packed. Cauterization is only used in some cases at the base of enlarged turbinates (these are on the opposite side of the nose from the septum and can cause obstruction if very large).


7. Which approach is preferred? Internal or external?

The approach depends on what part of the nose the surgeon needs to address. Internal approaches are used for most primary rhinoplasties.
